ALL FIRST TIME MED SPA suite bookings require a one-time 15-minute call with our Director. Please schedule your call below. At this time, we require all registered nurses booking the Med Spa to either be sponsored by a Medical Director or have FPA authority.

All bookings will require:

• Payment at the time of booking

• A copy of your current driver’s license or ID

• Your active license with IDFPR

• Proof of general and professional liability insurance

• * Weekly rate applies for a booking of a minimum of 5 days.
